Is this survey anonymous?

You will not be asked for your name nor your email address. In addition, reporting on any demographic or team will not take place if there are less than 5 responses. You may skip any question, including demographic questions.

Can anyone from my organization see my answers?

No person from your organization can identify individuals from their survey responses as survey scores are provided at the overall organization and team level but not at the individual level.  Please note that open text responses are provided verbatim.
